According to the Faculty of Sexual and Reproductive Healthcare:

"Alagille syndrome (AS) is an autosomal dominant disorder associated with abnormalities of the liver, heart, skeleton, eye, kidneys, and characteristic facial appearance. There is no direct evidence available regarding the contraceptive options for women with AS. The choice of contraceptive method may vary according to the signs and symptoms of AS (involvement of the hepatic or cardiovascular system).

Hepatic disease is a key feature of AS. Hepatosplenomegaly and hypercholesterolemia are common in patients with AS. The UK Medical Eligibility Criteria for contraceptive use recommends that in case of known hyperlipidaemias, use of any of the other methods of contraception, except combined oral contraceptives, outweighs the theoretical or proven risk (UKMEC category 2).

Cardiac symptoms, commonly associated with Alagille syndrome are pulmonary stenosis, atrial septal defect, ventricular septal defect, tetralogy of Fallot, patent ductus arteriosus, and pulmonary atresia .UKMEC suggests that any method of contraception can be safely used (and any method except combined hormonal contraception in complicated type) in the presence of valvular and congenital heart disease.

The CEU suggests that any method other than the combined oral contraceptive could be safely used but other medical conditions commonly associated with AS should be considered before prescribing contraception to such women and may limit the choice."
