We searched the NLH Learning Disabilities Specialist Library and the PsycInfo and Medline databases but found no risk assessment scales or instruments to evaluate the likelihood of a person with learning disabilities experiencing placement breakdown or crisis.

However, a paper by Broadhurst and Mansell details individual and organizational factors associated with breakdown of residential placements for people with intellectual disabilities should be of relevance. The main findings of this paper can be read by following the link, given below [1]
